An automation exception checklist defines what happens when a workflow cannot safely finish. A useful automation needs a visible failure queue, a human owner, and a recovery path alongside its successful path.
Download the editable CSV worksheet. Open it in your spreadsheet application, keep the instructions, and complete the blank entry, owner, due date, and evidence columns. This is a blank planning tool; the example below is illustrative.
When to use this worksheet
Use it with the manager who owns this workflow and one person who performs the work. Bring a recent completed case and a recent exception. Compare the worksheet with those records before adopting it as a team standard. If an existing system already stores these details, use its fields rather than maintaining two competing records.
Template fields and completion checklist
| Field | What to record |
|---|---|
| Trigger and input | Define what starts the workflow and required data. |
| Validation | Check permissions, required fields, and acceptable formats. |
| Failure categories | Separate temporary failures from invalid input or business exceptions. |
| Human owner | Assign monitoring and backup coverage. |
| Recovery action | Define retry, correction, manual processing, or cancellation. |
| Audit trail | Record status, attempts, decisions, and final outcome. |
How to put the process into practice
Map the manual process and identify actions that affect customers, money, or sensitive records. Decide which steps need human review.
Test missing fields, unavailable services, duplicates, and timeouts in a controlled environment. Verify that failures create a visible task rather than silently disappearing.
Define safe retry limits and prevent repeated execution from creating duplicate actions. Review the exception queue during the pilot before expanding volume.
Worked example
Illustrative example: a signed agreement normally creates an onboarding project. A timeout occurs after the project is created but before confirmation returns. A retry checks the agreement's stable identifier before creating anything else, then routes an ambiguous result to a person. This prevents two teams from onboarding the same customer.
Common mistake to avoid
An email alert alone is not an exception queue. Assign a state, owner, age, and closure record so unresolved failures remain visible after the notification is read.
What to measure after implementation
Report failed runs, duplicate actions, oldest unresolved exception, and recovery effort. Compare the total operating effort with the manual baseline.
Review the first completed worksheet
Before expanding this process, have the responsible manager check one completed record against the original evidence. Confirm that another team member can understand the next action without asking the author to explain it. Record any missing information as a revision to the worksheet. Choose a review date based on how often the workflow runs; a weekly process gives feedback sooner than a quarterly one.
